How Photo Search Works Here
OnlyFans itself has no photo search. It only looks up creators by exact username, which is no help when a picture is all you have โ and that is the usual situation: a screenshot from a clip, a teaser someone reposted without crediting the account.

Why Not Just Use a Regular Reverse Image Search
General reverse image search answers a different question. It looks for pages where that image file appeared โ mirrors, reposts, scraper sites โ and it has no idea which OnlyFans account, if any, belongs to the person in the photo.

Getting Better Matches
If the first attempt disappoints, the photo is usually the reason rather than the index:
